Health and Welfare Plan Eligibility
Uniti health and welfare benefits are available to U.S.-based employees regularly scheduled to work at least 30 hours per week and their eligible dependents. Eligible dependents include the following:
- Your spouse, which means a person recognized as married to you by the state, possession, or territory of the United States in which you were married, regardless of where you live. If you were married in a foreign jurisdiction, your spouse means a person recognized as your spouse under the laws of at least one state, possession, or territory of the United States, regardless of where you live.
- Your children through the end of the month in which they turn age 26, regardless of their marital status, regardless of student status and whether or not they live with you or you provide any of their support.
- Children for whom the Plan is required to provide coverage under a Qualified Medical Child Support Order (QMCSO); and
- Adult children who are mentally or physically disabled (as determined by the Social Security Administration), who live with you, and who are primarily dependent on you for support (you must provide appropriate documentation to the medical carrier) provided that the child was disabled and covered on the plan prior to age 26.
Children include the following:
- Biological children
- Stepchildren
- Legally adopted children
- Children who are placed in your home for adoption
- Any other children who are your tax dependents
- Children for whom you are appointed as legal guardian who are chiefly dependent on you for support and maintenance
Ineligible dependents include but are not limited to domestic partners, ex-spouses, parents, and grandchildren.
When enrolling, you will need to have your dependents’ Social Security numbers (SSNs) and birth dates available. You will also need to provide required documentation, like a birth certificate, marriage license, and/or current year tax filing, to enroll dependents who were not previously verified. Submit copies or scanned copies only. Documents will not be returned to you. Since you may need to order documents from vital records/your local clerk’s office, please begin collecting these documents immediately. Uniti does not retain copies of documents submitted.
